Making Referrals To Hogares

Contact our Intake Coordinator to initiate a referral

We help young people cope with the problems and pressures of growing up in a fast paced, high-pressure world.

Basic referral information:

  1. Client information: name, date of birth, social security number, ethnicity
  2. Medicaid status if known
  3. Access Center (Intake)
  4. Client’s current location
  5. Legal Guardian information: name, address, phone
  6. Juvenile Probation and Parole Officer, Guardian Ad Litem, Attorney contact information – as applicable
  7. Reason(s) for referral
  8. Level of care/services being requested (or may be determined by assessment)

What Levels of Care does Hogares provide?*

24-Hour Care:
Residential Treatment Care
Treatment Foster Care
Transitional Living Services

Outpatient Services:
Day Treatment (3-4 hrs/day Mon-Fri)
BMS – Behavior Mgt. Services
Case Management
Outpatient Therapy
Respite Center

Who do we serve?

We help young people cope with the problems and pressures of growing up in a fast paced, high-pressure world…problems such as difficulties at home or at school and drug and alcohol use.

Who can make referrals?

Parents, school personnel, therapists, mental health workers, social workers, probation officers, anyone who has an interest in the client.

Private insurance

Medicaid:
NM Medicaid (exempt)
Cimarron Salud
Lovelace Salud
Presbyterian Salud

Non-insured- can go on sliding scale fee
